Regulated cell death (RCD) stands at the crossroads of cellular integrity and disease, orchestrating essential processes that preserve tissue homeostasis, guide development, and dictate responses to stress. Far from being a passive consequence of damage, RCD is an actively regulated and highly conserved mechanism, one whose dysregulation fuels the pathogenesis of cancer, chronic inflammation, neurodegenerative diseases, and muscular atrophy. Apoptosis, the classical and best-characterized form of RCD, ensures the silent clearance of compromised cells. Yet, its evasion of malignancies has propelled a paradigm shift toward uncovering non-apoptotic death modalities. Ferroptosis, driven by iron-dependent lipid peroxidation, has emerged as a potent vulnerability in apoptosis-resistant cancers, such as acute myeloid leukemia, with p53 serving as a critical modulator of this process. Meanwhile, the recent identification of cuproptosis, triggered by intracellular copper accumulation, has opened new investigative avenues in disorders such as skeletal muscle atrophy, highlighting the pathological relevance of metal dyshomeostasis. RCD is not an isolated event, it profoundly shapes and is shaped by the immune landscape. Innate sensors such as NLRs dictate inflammasome activation and the immunogenic profile of dying cells, influencing disease tolerance and immune surveillance. Central transcription factors, including MYC and p53, act as master integrators of death and proliferation signals. Heat shock proteins, particularly HSP70, safeguard cancer stem cell survival and modulate apoptotic thresholds. From a translational perspective, targeting RCD pathways represents a high-value strategy across disease domains. In cancer, combining radiotherapy with ferroptosis inducers offers new hope against resistance. In neurology, modulating the endocannabinoid system and autophagy may confer neuroprotection. Mitochondrial dysfunction, at the core of multiple RCD forms, underscores the centrality of metabolic control in cell fate. RCD is not merely a biological endpoint, it is a therapeutic frontier.
